Over 1.5 million Australians are estimated to experience an adverse reaction to medicine each year. Medication-related incidents contribute to 190,000 hospital admissions.
This conference will focus on hospital medication safety systems and governance in the context of (NSQHS) Standard 4 – Medication Safety.
It will examine the ways in which hospitals can shape their processes to be successful in accreditation, and also ensure the highest levels of clinical care and safety.
Medication safety is not simply about compliance for accreditation – it is about building sustainable best practice systems to reduce medication incidents and ultimately benefit the patient.
This conference will address how to:
+ Implement resource and time-management strategies to strengthen medication management
+ Foster a culture that drives standardised systems and continual process improvement
+ Strengthen communication to improve medication safety at transitions of care
