Streamlining discharge processes
This conference will address the challenges associated with preventing readmissions via effective discharge planning and service integration. The National Safety and Quality Health Service Standards have influenced the way consumer engagement, medication management and clinical handover are managed in hospitals. This conference will cover strategies for ensuring compliance to internal policy and national standards.
Recent research out of the US has evidenced a clear link between complicated and fragmented transfer of care and readmission rates. This highlights the importance of effective clinical communication throughout the patient journey.
